First Time Adult Leader Recommendation Form

First time leaders are required to be known by their church for a period of 6 months and to have a Leader Recommendation Form completed by a member of their pastoral staff before they can serve with Flower City Work Camp.  Church Representatives will conduct this step using the form below and will submit the completed forms to the FCWC Ministry Director Jill Herman in a timely fashion.

Download this file to your computer, have a pastor fill it in and save it for each 1st Time Adult Volunteer.
